The seven north east London clinical commissioning groups (CCGs) – City and Hackney, Newham, Tower Hamlets, Waltham Forest, Barking and Dagenham, Havering and Redbridge – will become a single organisation called North East London CCG from 1 April 2021. Transforming Care Partnership plan

This three year plan, which you can download below, sets out our vision and confirms the commitment of the Barking and Dagenham, Havering and Redbridge (BHR) Transforming Care Partnership (TCP) for improving the care and support available for children, young people and adults with a learning disability and/or autism, who display behaviour that challenges – including those with a mental health condition.

Across BHR we have already made excellent progress in moving away from inpatient care and developing supportive community provision, such as the support given to client AB in this case study.

We will not stand still, however, as we recognise there is much more to do. The work to be taken forward through this programme will be wide-ranging, and we will continue to work closely over the coming months with our partners, including people who have a learning disability and/or autism, the BHR Learning Disability Partnership Boards (LDPBs), local third sector organisations, national organisations in the health and care system (such as Health Education England) and all members of the Transforming Care Partnership.
